With changing times, practices change. What was once miniscule becomes mainstream. The coronavirus pandemic has forced upon us and for the better, certain new ways of interaction. Telemedicine holds immense value in the days to come. During a pandemic, when the city is in a lockdown and stepping out is nearly impossible, cardiac teleconsultation becomes a saviour. Patients can consult their doctor, discuss their problems just as they would in the clinic and receive valuable advise and digital prescriptions, all using digital technology. Only patients who may need to be seen physically could be identified and called upon to do so, saving the risk of infection and hassle of travel they may otherwise face. Q. Will the teleconsultation experience be similar to a clinic consultation?

 A. While it is true that you might have never experienced a cardiac teleconsultation before, it is quite similar to a clinic consultation except for the physical examination. All aspects of a consultation including history taking, evaluation of medical records and previous prescriptions, analysis of available investigations, formulating a diagnosis, ordering investigations and generation of digital prescriptions are done during a teleconsultation. All questions pertaining to the symptoms, illness and treatment can be discussed in real time. A video consultation also allows for inspection of the patient and seeing your treating physician can also be reassuring.

Q. How can a medical consultation be complete without physical evaluation?

A. Physical examination is an important part of a medical consultation. However, it may not always be necessary when symptoms are insignificant. In patients who seek a routine follow up, symptoms and lab investigations are enough for assessment. Home blood pressure monitors and in some cases wearable monitors can provide the vitals, although it is not always necessary. At times, the patient may require to be seen and only these few patients can be called upon to do so thereby avoiding the unnecessary risk of travel and exposure in the times of a pandemic.

Q. I cannot come to the clinic. Can’t I communicate with my doctor on WhatsApp or a phone call ?

A. While the doctor may provide guidance on call in cases of emergency, a cardiac teleconsultation with an appointment provides a dedicated time for discussing your medical problems and getting an advice with a prescription. 

Q. Will I receive a prescription after the teleconsultation? Can it be used to purchase medicines?

A. After each cardiac teleconsultation, a medical record will be generated. It will include the patient details, the medical details, diagnosis, investigations ordered, a prescription and follow up details. The record will be shared with the patient in the form of a PDF file in the form of a link using SMS on the patient’s registered mobile number. The digital prescription follows the regulatory guidelines and can be used to purchase prescriptions from both online and offline pharmacies.

Q. Are the charges of a teleconsultation same as that of a clinic consultation?

A. The charges of a teleconsultation are 20 percent less than that of a clinic consultation. It also saves the time and money required for travelling to the clinic for consultation.

Q. Is the process of booking a teleconsultation cumbersome?

A. The process of booking an appointment for teleconsultation is very simple.

Q. Will the consultation be though a telephone call or always a video consultation?

A. Most cardiac teleconsultations will be done through a phone call. The previous medical records and investigations may be uploaded on the app while fixing an appointment. If necessary, a video consultation may be scheduled. 

Q. How long will the consultation last?

A. A typical consultation takes less than 15 minutes. However, if necessary it may last longer until all your questions have been answered. 

Q. Can teleconsultation continue even after it is safe to travel to the clinic?

A. Indeed. Cardiac teleconsultation is an excellent way of seeking a medical consultation especially for patient who have difficulty in travelling and want to maintain continuity of care. Similarly, for patients who seek a routine follow up and are asymptomatic, teleconsultation is a great way to continue treatment.
